HOW DO PHYTOSTEROLS WORK?

PHYTOSTEROLS AND ITS MECHANISM OF ACTION

Phytosterols are natural food ingredients present in plants and it has been known since the 60’s that they have the ability to reduce LDL cholesterol. According to multiple meta-analyzes, the daily consumption of 2.000 mg of phytosterols reduces cholesterol around 10 and 15% (Demonty, 2009, AbuMweis, 2008, Ras, 2013). The proposed mechanisms for cholesterol reduction are (De Smet, 2012):

  • Phytosterols compete with cholesterol for its incorporation into the micelle at the intestinal level, which results in less cholesterol entering the epithelial cells of the small intestine, called enterocytes.
  • The cholesterol that reaches enter the enterocytes must be esterified to go on to form the lipoproteins that then go to the blood. The phytosterols compete again with cholesterol to avoid this step, inhibiting an enzyme called ACAT.
  • The phytosterols, which does not get into the blood, are expelled again to the intestinal lumen. In this process, an amount of unused cholesterol is also expelled.
  • Phytosterols also tend to co-crystallize with cholesterol in the intestine, preventing it from being incorporated into micelles (Rozner, 2006), and recently a mechanism of cholesterol secretion has been described directly from the circulation to the intestinal lumen (De Smet , 2012).
